Payment and Policies
Insurance
I am able to bill BCBS of MA or GIC/Unicare for physical therapy services according to your coverage benefit.
I do not take insurance for Functional Medicine or the Chronic Pain Program.

If you are covered for physical therapy, you may still be in your deductible or have a co-insurance payment that needs to be paid out of pocket. If you are unsure of the amount of your co-payment or co-insurance, please call your insurance provider for details.
Payment Methods:
If you do not have either of the insurances listed above, I provide private physical therapy services on a direct pay basis. I try my best to strike a balance between making it affordable for you while also receiving fair compensation. Please see below for fees. Payment is due at time of service. Cash, Credit/Debit Card, or Venmo is accepted. You may qualify for sliding scale payment - please see the sliding scale application.
To make things easy on all ends, please update your personal profile through Square to include a valid credit card number for billing or download and set up the Venmo app onto your phone.
Fees:
Physical Therapy

Evaluation: $150.00

Physical Therapy Treatment: $90 for 45 min (unless you qualify for a sliding scale).
Functional Medicine:

Initial Evaluation + 1 care planning follow-up visit: $450
Initial Evaluation usually takes 2-3 hours, sometimes more depending on the situation
Care planning follow up visit: 1 hour

Treatment session: $100 for 60 min (unless you qualify for a sliding scale).

Cancellation Policy
Kindly provide 48 hour notice if you need to cancel or reschedule your visit. I may not be able to fill your spot with shorter notice cancellations. This might cause another person who really needs the treatment to miss an available opening, as well as lost income for me. Short notice cancellations will be charged in full.
